NOTES:

1. Please complete all information.  ONE ENTRY FORM PER PLAYER, you can send multiple entries in the same envelope.
2. Entry Fee is $25 per player.  NO CASH in the mail.  Make your check payable to “VCF”.  Entry Fee must be paid (received) by 9PM Friday, March 7th.
3. Each player must be a current member of the U.S. Chess Federation (USCF).  You can join the USCF at the tournament during Late Registration, but it is FAR EASIER and PREFERABLE for you to join online at www.uschess.org no more than 2 weeks before the tournament.  If you join online, please bring a copy of your new USCF Membership Card with you.
4. If you prefer to pay after you arrive at the tournament, you must do so by 9PM on Friday March 7^th or you risk not being paired for the 1^st Round of the tournament.  Cash or Checks only, NO CREDIT CARDS.
5. Complete and mail this form and your payment to the address shown below.  Your entry must be post-marked by Monday March 3^rd.

6. After we receive your entry, we will send you a confirmation email, or give you a phone call.
7. Registrations will be posted on the Tournament Web Site (www.vachess.org) as soon as possible after we receive them.  A registration is not “complete” without your payment in full, including USCF membership dues (if required). 
8. Players not paid in full when the Registration Desk closes at 9PM on Friday March 7^th will not be paired for Round 1 of the tournament.
